IPTAAS - Isolated Patients Travel and Assistance Scheme

A partial reimbursement scheme for people who attend the nearest treating specialist for treatment or consultation over 100km from their door. This is not a means tested scheme. If you are not on a pension there is a contribution of $40 per claim. You may also be elligable for assistance if you are travelling a cumulative distance of 200kms or more in a week. For a reimbursement calculator click HERE and scroll down. The amount is calculated is an ESTIMATE ONLY. The reimbursement amount will be finalised on assessment of your claim. Advance Payment Claim (2 pages)

To be completed by the patient  which involves your personal details and bank accounts

Referring Doctor Form

To be completed by the referring doctor (person sending you over 100kms to receive treatment)

You will need to ask the doctor to complete this form

This form only has to be completed once a year if it is to the same medical specialist

Claim Form

To be completed by the patient and/or carer and the specialist you are seeing i.e Oncologist, Radiotherapist etc. You will need to ask your specialist to complete this form.

Travel Diary

To be completed by the specialist you are seeing.

This is used if you have subsequent visits in a small time frame as a log book of your travel and/or accommodation. In this situation, the diary becomes your claim form.
