Nature of Service and Processes

- All services are provided by volunteer drivers who have no medical training. All clients who have a significant disability or medical condition must be accompanied by their carer.

- Before any service is provided an assessment is undertaken

- The service is funded for 3 vehicles which can accommodate up to 3 people including the driver.

 

Eligibility

- Frail and older persons experiencing moderate, severe or profound disabilities and their carers

- Young people experiencing moderate, severe or profound disabilities and their carers

- Such other persons as agreed upon by the State Minister and the Commonwealth and their carers

The HACC Program also aims to address the unique requirements of special needs groups within the target population, including:

- Aboriginal and Torres Strait Islanders (ATSI).

- People from non-English speaking backgrounds (NESB).

- People living with dementia.

- People experiencing geographical isolation.

- People experiencing financial disadvantage.

This also includes those who are;

- Receiving the aged Pension

- Self-funded retirees included

- Veterans affairs

- Disability Support Pension (includes a wide range of mental/physical/intellectual disabilities but this need to documented by health providers)

- Carers who are included if documented by health providers

- Living in isolated communities

- Requiring dialysis treatment (Waiting on further information)

o Armidale

o Glen Innes

o Inverell

o Tenterfield

o Brisbane

o Toowoomba

o Coffs Harbour

o Lismore

o Tamworth

* A small fee is charged e.g. Glen Inness to Armidale $30.00 Return. The fee is waivered for those experiencing financial hardship

 

Referrals

Referrals are accepted from any source in the community, including doctors, domiciliary services, social workers, hospitals, the Aged Care Assessment Team (ACAT), another HACC service, carers, relatives, friends, neighbours or even the person him/herself. The referrer will be asked for information that is necessary in determining the applicant's eligibility for service.

An assessment is then used to determine the extent of need that person has for assistance. Most assessments are conducted in the applicant's home and s/he can request an advocate or interpreter to be present. HACC services receive government funding and most services charge service users a proportion of the cost. However, the fees policy must be fully explained to the applicant and no person is to be denied service if they cannot afford to pay. Of course, this is subject to the availability of funds in the service to subsidise this amount.
